The offline process of name
change or correction in the PF account is very simple, so don't worry. The
offline process of correction in PF account requires a joint declaration form
from you and your employer along with suitable supporting documents. The
necessary correction will be made once the correction is submitted to the EPFO.
Problems due to incorrect details in EPF account
Incorrect details in your PF
database can create problems at the time of PF withdrawal. Any incorrect entry
of data in the PF’s record during enrolment can cause rejection of PF
withdrawal. Therefore, it is very important to ensure that all details recorded
at the time of the registration are correct. For smoother approval PF
withdrawal, all the details given on the form should match the PF database.
Rules for name change in PF account
If there is a need to change
or correct your name in PF database, you alone can’t give an application to
EPFO for the same. It requires a joint declaration form from you and your
employer along with suitable supporting documents, to change or correct your
name in the PF database.
Documents required for name change in PF account
The supporting documents may
be any of the following as identity proof in order to make the necessary

1. PAN Card
3. Passport
4. Driving license
5. ESIC Identity Card
6. Aadhaar Card
7. Bank Passbook Copy / Post
Office Passbook.
8. Ration card
9. Any school/education
certificate
10. Certificate issued by
Registrar of Birth & Death.
11. Certificate based on the
service records of the Central/State Government Organization.
12. A copy of
electricity/water/telephone bill in the name of the claimant.
13. Letter from a recognized
public authority or public servant verifying the identity and residence of the
member to the satisfaction of the competent authority.
EPF Name Change / Correction Form
There is a prescribed format
for name change/correction form. This form is a simple document in the form of
a letter. The sample of name change/correction form in EPF account is shown in the
below image.
In the above form, you have
to fill the address of the Regional PF Commissioner, your name, and your
company name. You need to fill the correct and wrong particulars in the table
with three columns which are needs to be changed/corrected and other columns
should be left blank. You also need to enter your name and sign it. The
authorized signatory of your company also has to enter his/her name, sign the
application and affix the establishment seal on it.
Corrections that can be made in your PF account using this form
This form can be used for
making the following six corrections.
1. Correct the name.
2. Correct name of the
father or husband.
3. Correct PF or EPS account
number.
4. Correct date of birth.
5. Correct date of joining.
6. Correct the date of
leaving.
Steps involved in Name change/correction in EPF Account
1. Fill the prescribed form
with all the required details.
2. Sign the filled form.
3. Get it signed and stamped
by your employer.
4. Attach a self-attested
copy of any of the documents mentioned above with the application.
5. Now, the application is
sent to the regional PF commissioner by your employer.
Check name and date of birth in PF account
To make sure, you should
check your name and date of birth in your PF after the application of EPF name
change. Just login into UAN member portal. Download your passbook which
contains your name and date of birth. Check for any mismatch in name and date
of birth in the passbook.
Name correction in EPF Account due to the marriage
Often, women add a surname
or title after the marriage. If they change their surname in their office
records and bank records then they should also change their name in the EPF
database. For this, just fill the name change/correction form and attach a copy
of the marriage certificate. The name in a bank and EPF database should be the same
for EPF withdrawal.
